A groundbreaking study is challenging the status quo in gout treatment, but not without stirring up some medical controversy. In the study, 152 participants with uncontrolled gout were given either a low dose of oral methotrexate (15 mg) or a placebo, along with folic acid and IV pegloticase, for up to 52 weeks. Uncontrolled gout in this context meant patients had high uric acid levels, frequent flares, visible tophi, and poor response to standard urate-lowering therapies.
But here's where it gets interesting: Despite concerns about methotrexate's potential impact on kidney function, especially in CKD patients, the study revealed that both groups experienced stable renal function. The estimated glomerular filtration rate (eGFR) even showed a slight improvement in the methotrexate group compared to the placebo group (4.6±1.4 mL/min/1.73 m2 vs 1.1±4.2 mL/min/1.73 m2).
And this is the part most people miss: Not only did the methotrexate group maintain their kidney function, but they also had similar or better outcomes in other areas. Most patients in both groups maintained or improved their CKD stage, and albuminuria grade was not negatively impacted by methotrexate. Additionally, adverse events (AEs) were comparable between the two groups.
However, a closer look at the data reveals a potential caveat. While the overall safety profile was similar, more patients in the methotrexate group experienced mild increases in alanine aminotransferase levels compared to the placebo group (13.6% vs 6.3%). This finding raises questions about the long-term effects of this combination therapy on liver health.
